When you think of care workers, you tend to picture the professional who visits your home, the friendly face, the helping hands. But what you may not always see is the team of people working quietly behind the scenes to make sure everything runs as it should.

If you're considering switching NDIS providers, this hidden support might be exactly what you're looking for, not just quality care on the day, but the confidence that comes from knowing there's a consistent team who understands your goals and how you want to be supported.

It's natural to wonder what will really feel different when you're thinking about changing your NDIS provider.

For many people, it's not just about the support they need today but what happens tomorrow when there's an urgent problem and they need to speak to someone asap, what happens when their regular care worker doesn't show up, who to speak to if they have to move home or there's a change in their needs.

Things like knowing who to talk to, getting a response when you reach out, and not having to repeat your story every time make a big difference.

Our approach, which we call our Senior Care Worker model was built and refined over 22 years of listening to our customers and families, to learn what worked well, what we could improve and understand what they wanted more of.

Your Support Team, Not Just a Roster

One of the things we hear often from people considering a new provider is how unsettling it can feel when support workers keep changing.

It's hard to build trust, feel comfortable, or make real progress on your goals when you're constantly having to reintroduce yourself.

That's why our Senior Care Worker Model takes a different approach in matching your needs to a care team including support from a Senior Care Worker and a dedicated Care Worker Circle with more than one Care Worker to provide consistent, reliable quality care.

Service Engagement – Here To Help You Get Started

At the start of your care journey with BCD, you'll be welcomed by our Service Engagement team who are here to help you get set up.

When you start with BCD, we will ask you a few questions about you and what you would like and what your needs are.

  • This is so we can make sure we meet your expectations and match you with a care team with the skills that are needed to give you quality service.

  • Our Service Engagement team explain how we work, help you with any paperwork, and direct you to the next step.

It's all about giving you a warm welcome and making sure you feel informed, supported and confident from day one.

 

Your Go To Key Contact - Senior Care Worker

One of the first BCD team members you'll meet will be your Senior Care Worker. As your key contact, they will help you make the most of your services and your Care plan with BCD.

This includes things like:

  • Care planning with the people who provide the supports and guiding others.

  • Spotting risks early - like noting allergies or medical conditions and making sure your Care Worker Circle is informed and prepared to support you.

  • Arranging or completing assessments with you.

  • Regular check in throughout the NDIS plan period to ensure things are tracking well.

  • Tailoring how they contact you to what suits you, whether this is a phone call, on email or via a text message.

  • Monitoring NDIS funding so we can ensure the best use of NDIS funds.

  • Planning what supports you need and making sure it all happens!


A big part of the care planning process that the Senior Care Worker does is spotting potential risks early and putting the right steps in place to keep you safe.

That might be something straightforward, like making sure your Care Worker Circle are aware of an allergy, or medical condition such as diabetes to more complex planning to prevent things going wrong down the track.

Their focus is on keeping everything on track and overseeing your Care Plan. This is especially important for your NDIS plan review preparation or in the event your circumstances change and you need to request unscheduled plan review with the NDIA.



Your Care Worker Circle - Familiar Faces You Can Count On

Alongside your Senior Care Worker, will be your Care Worker Circle who will help you with your daily support needs.

  • Your Care Workers are carefully matched to your support needs.

  • Your care team will include two or three regular Care Workers, so you get consistent care that fits around your needs and goals.

While they're the ones you'll see most often, they're backed by a whole team of people working together to achieve your goals so that every visit feels consistent, thoughtful and right for you.


Care Central - One Point Of Contact, For When The Unexpected Happens

Behind the scenes, our Care Central team is available to help adjust your rostered services and make sure you're always supported.

  • When the unexpected comes up, like a last minute specialist appointment or an urgent request, the Care Central team are here to help you adjust your schedule and keep things on track, even at short notice.

  • You won't be left chasing updates or waiting around.


In House Registered Nurses - On Hand When You Need Them

At BCD, we have an in-house clinical nursing team of registered nurses who are part of your care team where a clinical need is identified.

They work alongside your Senior Care Worker and Care Worker Circle to:

  • Monitor health needs

  • Address any health issues promptly

  • Where a clinical need is identified they help manage care plans to make sure the support you receive is right for you.

Clinical nursing care focusses on preventative care to avoid hospitalisation or during extended periods of illness.


While you may not meet everyone in person, each BCD team member is part of your village when you start your care journey with us.

They play a quiet but important role in keeping your support consistent, reliable, and shaped around what matters most to you.
