Support At Home: How Our Clinical Nursing Team Delivers Care
Did you know that a key feature of the new Support at Home program is fully funded clinical nursing services delivered by registered nurses in your own home.
Starting 1 November 2025, the Australian Government's Support at Home program will replace Home Care Packages and Short-Term Restorative Care programs.
This post introduces the dedicated BCD clinical care team behind your care plan, explains how care management works and shows why you won't pay for clinical services under the new Support At Home program.
At a glance
How our BCD clinical nursing team supports you: Registered, enrolled and assistant nurses work with your Senior Care Worker and Care Worker Circle to provide clinical assessments, wound care, catheter care, continence reviews and specialist assessments, helping you stay well at home and avoid unnecessary hospital visits.
How care plans work: Plans are documented and reviewed annually or if there has been significant change in your health condition or circumstances where you become more at risk. Care plans are linked to your goals and the funding level.
What types of clinical nursing and assessments are provided.
One Care Partner
Under the new Support at Home program, services are now grouped into three categories: clinical supports, independence and everyday living.
Clinical supports include nursing care, continence care, occupational therapy and physiotherapy. These services are delivered by registered nurses and allied health professionals.
Because clinical supports are fully funded, you won't pay any out‐of‐pocket costs for nursing or allied‐health services.
From 1 November 2025, each participant will now have a single provider responsible for delivering all approved services.
At BCD, our clinical nursing team is a vital part of our Senior Care Worker model, a team approach we've built from over 22 years of listening to our customers and shaping a village of care with you at the centre.
How Our Nurses Support You
When you start your aged care journey with BCD, you will have a Senior Care Worker assigned to understand your health needs, goals, preferences and existing supports, and to arrange services that align with them.
When a clinical need is identified our in house clinical nursing team will also form part of your Care Circle.
Your nursing team will collaborate closely with your GP, allied health teams and loved ones to create a personalised plan that supports your health and wellbeing.
They work hand‐in‐hand with your Senior Care Worker and Care Worker Circle so that support is consistent and tailored to your care plan.
By focusing on early intervention, they help you stay well at home and avoid unnecessary hospital visits or admissions.
Here are some of the ways our nurses support you:
Clinical assessments: These are in‐depth reviews of your health story. Our nurses take time to understand your medical history, current health and lifestyle so they can create a plan that helps you feel better today while preparing for tomorrow. Early action prevents small issues from becoming larger problems.
Wound care – healing in the comfort of home: Recovering from surgery or managing a wound is always easier when you are in your own space. Our nurses bring wound care to you with patience and reassurance, creating a calm environment that supports healing.
Catheter care – supporting daily health needs: Living with a catheter can feel daunting, but our nurses make it manageable. By keeping everything clean, comfortable and simple, they reduce the risk of infection and help you focus on living well.
Continence reviews – dignity and confidence every day: Being comfortable and confident matters. Our continence assessments help us understand your needs and create a plan that works for you, whether that's improving or managing bladder or bowel control.
Specialist assessments: Our clinical nurses can provide a wide range of specialist assessments and monitoring. These include falls risk assessments, nutritional reviews, pain assessments (including for advanced dementia), diabetes management, medication reviews and administration, oral health checks, pressure ulcer risk assessments and vital signs monitoring. Each assessment is carried out with care and attention, explained clearly, and always connected back to your overall wellbeing.

Our customer stories show how the small changes made the biggest difference.
This was the case for one of our customers, Xiaolin who needed urgent nursing support and a nursing team who could communicate clearly without shared language.
From the first week, our in-house Registered Nurse, Kim visited on a steady schedule, gently managing Xiaolin's wound care with careful cleaning, dressing and monitoring.
That consistency mattered, helping the wound heal as expected and preventing complications.
Because English isn't Xiaolin's first language, BCD nurse Kim adapted how they connected using simple visuals, gestures, slow demonstrations and plenty of eye contact.
Over time, a calm routine took shape: the same friendly face, the same steps, the same time of day. Pain eased, confidence grew, and Xiaolin's family felt reassured knowing exactly who was coming and what would happen at each visit.
Preventative care that changes lives - Carol's Story
When managing her health at home became too hard, Carol moved into one of our Supported Independent Living (SIL) homes with our Clinical Nursing Team leading the way.
From day one, the BCD nursing team stepped in to provide round-the-clock clinical care. They check Carol's blood glucose levels, review observations from the Care Worker Circle, adjust care in consultation with her GP, and step in quickly when something changes.
Small signs are caught early, a dressing that needs changing, a pattern in night-time readings, a new symptom to watch, so issues are treated before they become emergencies.
Since the BCD nurses took the lead, Carol's days are steadier and safer. Hospital trips are rare, medication is on track, and her care plan is updated as her needs shift.
Most importantly, Carol knows a familiar nurse is always there to listen, explain and act which makes life feel calmer for her and her family.

What is an aged care provider?
An aged care provider delivers the care services to you. Whether this is nursing care or other support at home that you need help with for example, showering, meals or transportation.
Can I receive care from more than one aged care provider?
From 1 November 2025, a single aged care provider will manage and deliver all your services under the Support at Home program.
For example, If you need nursing support at home and help with showering and transportation, you will have one provider assigned that will deliver all these services. Your provider will also assist you to arrange and source any required assistive technology and home modifications you may be approved for.
