An important part of our approach is the Senior Care Worker role. From day one, your Senior Care Worker adds an extra layer of personalised care, working alongside your dedicated Care Worker circle.

They're there as your go to person and key contact, ensuring everything runs smoothly behind the scenes. They deeply understand your story, your support needs and preferences, so you never feel like 'just another name on the roster.

Your Senior Care Worker regularly checks in, listens closely, and communicates clearly and proactively, always with your comfort and goals in mind.

They make sure everyone in your Care Worker circle understands your needs and what's important to you, so they’re ready to provide support in exactly the way you prefer.

Your Senior Care Worker helps by:

  • Working closely with your Care Worker circle, ensuring everyone's clear about your goals and the best ways to support you.

  • Proactively identifying potential risks or needs early-whether that’s sharing essential health information, such as allergies or medical conditions, or arranging preventative measures to avoid issues later on.

  • Completing internal BCD assessments and making sure they're completed promptly, so you always have up-to-date information for your support plan.

  • Regularly checking in with you throughout your NDIS plan period, ensuring your supports remain aligned with your goals and proactively adjusting supports if needed.

  • Communicating in whatever way suits you best-phone, email, or text-so staying in touch feels easy and comfortable.

  • Monitoring NDIS funding so we can ensure the best use of NDIS funds. This ongoing oversight is especially valuable when preparing for your regular NDIS plan review or when life changes unexpectedly, and you need support with requesting an unscheduled review from the NDIA.

  • Anticipating future support requirements and making sure everything's arranged well ahead of time.
